This form is to be used by SHC members that would like to submit a formal complaint against an individual involved with Sturgeon Hockey Club.
Before a formal complaint is filed, individuals should attempt to resolve the complaint through the appropriate channels and protocol as documented in Sturgeon Hockey Club's policy and procedures.
Anonymous complaints will not be accepted. Your information will be kept confidential after filing the complaint.
